ON, MA – Hilltown Community Health Centers was a proud recipient of Florence Bank’s 2015 Customers’ Choice Community Grant Program. In a ceremony on March 9, Eliza Lake, Executive Director was presented with a check for $798.00 by Monica Curhan of Florence Bank. HCHC was one of fifty-seven non-profit organizations that received enough votes from bank customers to be given a grant. The money received will be put to good use by supporting the Health Outreach Program for Elders (HOPE) program by allowing us to continue the HOPE nurse who serves elders in the northern hilltowns who can’t get visiting nurse services, have a very hard time getting out of their homes, and are often very isolated. HOPE visits provide elders with needed medical care such as vital sign checks, immunizations, blood draws and medication management, along with the support that allows them to access other needed community and health programs.
