To make an appointment, please call the health center or use the button below to fill out an online form.

Visit Our Locations page for locations and phone numbers. Our receptionists are happy to assist you!

Bring the following with you to your appointment:

  • Health insurance cards
  • Health insurance co-pays, if one is required
  • A photo ID (driver’s license, passport, etc. to verify your identity)
  • All prescriptions that you currently take
  • Any medical records along with your immunization records
  • The name and address of your pharmacy

We're Moving

Our Community Programs will be relocating from 9 Russell Road to our health center locations. Starting August 1, our Community Health Workers and Insurance Navigators will serve all patients and clients in the Huntington, Worthington, and Amherst health centers. This move will help better serve you and your family.
