You can have easy and secure access to your medical information online on MyChart. Gain access to your private health information and receive periodic updates and reminders from your primary care provider via email.

With the Patient Portal, you’ll have access to:

Provide us with your personal e-mail address and you’ll then receive an email from MyChart with login information. It’s that easy!

FAQ

The information in MyChart comes directly from your healthcare providers. Ask your doctor to correct any inaccurate information at your next clinic visit.

Parents and guardians can request proxy access to a patient’s MyChart account by contacting the patient’s primary care provider.

MyChart accounts can be linked so you can see all your information in one view. We call this Happy Together. Depending on your healthcare provider’s version of Epic, Happy Together may not be available to you yet. View a list of participating organizations.

Your healthcare providers take great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal usernames, and passwords. Unlike conventional email, all MyChart messaging is done while you are securely logged into MyChart.

We're Moving

Our Community Programs will be relocating from 9 Russell Road to our health center locations. Starting August 1, our Community Health Workers and Insurance Navigators will serve all patients and clients in the Huntington, Worthington, and Amherst health centers.
