Health Outreach Program For Elders

The Health Outreach Program for Elders (HOPE) provides preventative and health maintenance care to seniors in their homes for free.

HOPE provides elders in the Hilltowns in-home medical care, including:

  • Vital sign monitoring
  • Blood draws
  • Flu shots and immunizations
  • Medication management
  • Assistance completing health care proxy forms
  • Help arranging appointments with providers
  • Referrals for housekeeping help, and much more!

HOPE serves elders who have difficulty getting out of their home for medical services and are often very isolated. HOPE visits provide elders with needed medical care and the strong support that allows them to access other needed community and health programs to remain safely at home. HOPE nurses can also assist with helping to arrange appointments with physicians and specialists, as well as arranging referrals for housekeeping help.

All services provided by HOPE are free to participants who are health center patients, age 60 and over and live in the Hilltowns.

Programs are generously funded through Community Development Block Grants administered through the Pioneer Valley Planning Commission and the Hilltown Community Development Corporation, as well as Highland Valley Elder Services. Private donations are also received.
