Primary Care Physician

Hilltown Community Health Centers, Inc. (HCHC) is seeking a Family Practice Physician to join our growing multi-site primary care practice. HCHC is a federally qualified community health center located in Western Massachusetts. For over 65 years, our mission has been to provide accessible, affordable services to ensure the health of all Hilltown residents, regardless of their ability to pay. Established in 1950, HCHC provides primary medical care, general dentistry, mental health, optometry, and community outreach in four different sites. HCHC currently employs three talented physicians and six experienced nurse practitioners, and operates two rural primary care clinics and one urban primary care clinic, along with a school-based health center, in Western Massachusetts. Behavioral health and full dental services are offered at all sites; optometry care is also available in one location. A strong community services program provides a range of supportive services, including community outreach workers. The position involves care of adults and children, in an office-based environment, without OB or hospital responsibilities. On-call schedule shared among 10 providers. Ability to form collaborative relationships with nurse practitioners on staff a must. Occasional Saturday and holiday hours required. Working knowledge of rural services and needs a plus. EHR experience preferred. Spanish or Russian language skills are highly valued. Salary commensurate with experience with possible eligibility for state and federal educational loan repayment programs. Excellent benefits include 4 weeks’ vacation, paid sick leave, personal and holiday time, 2 weeks’ continuing education time with $2,000 for CE compensation costs, malpractice coverage, health and dental insurance, as well as payment of licensing renewal fees. The health centers are located in the beautiful foothills of the Berkshire Mountains in Western Massachusetts. We are a 30 minute drive from Pittsfield, Northampton and Westfield as well as other towns in the Pioneer Valley, which is also home to the 5 college area of Smith, Amherst, Hampshire, Mt. Family Nurse Practitioner

Behavioral Health Clinician- LICSW/LCSW/LMHC – Worthington/Huntington

Seeking part-time Behavioral Health Clinician to provide dynamic, integrated patient-centered care in a rural, primary care setting with possibility of working at more than one location. We are seeking an individual to work with individuals, families, behavioral medicine, addictions, and a wide range of issues from severe trauma to adjustment reactions. Must be comfortable working within a multi-disciplinary medical team serving underserved populations. At a minimum, candidates must be Masters level and have a valid MA license to practice. Development Director

Hilltown Community Health Center seeks a part-time Development Director for a mission-driven non-profit devoted to providing medical, dental, behavioral health, eye care services, and community programs to everyone, regardless of ability to pay. Position is responsible for full range of fund development activities for the organization. Specifically, grant writing/reporting with federal and state agencies, private foundations and corporations; coordinating and implementing the organization’s fundraising activities and other development activities as appropriate. Minimum three years’ progressive grant writing and development experience in a non-profit corporation required, and experience with federal grant writing strongly preferred. Proven effective communication, with strong writing and editing skills essential. Ability to write clearly, articulately, and persuasively with attention to detail. Experience working in deadline-driven environments preferred. Knowledge and familiarity with research techniques for fundraising prospect research. Raiser’s Edge experience a plus. Able to work well in a team environment, handle multiple assignments and meet deadlines.


Medical Director

Hilltown Community Health Centers, Inc. (HCHC) seeks a leader for a mission-driven non-profit devoted to providing medical, dental, behavioral health, eye care services, and community programs to everyone, regardless of ability to pay. The candidate will effectively coordinate and oversee all health center medical activities and is broadly responsible for patient safety. The new Medical Director provides quality, direct care to patients along with leadership to the medical department with respect to aligning the department activities with HCHC’s strategic vision of quality care. Helping ensure services can be carried out in conjunction with the organization’s strategic plan and mission. Must be a Massachusetts board-certified Physician or Nurse Practitioner, with a minimum of five years of post-certification practice experience. Must have appropriate management experience and/or training. Must be committed to community health. Must also have a strong commitment to the mission of the organization. Good interpersonal skills also necessary.


Clinical Department Assistant

Hilltown Community Health Centers, Inc. (HCHC) seeks a Clinical Departmental Assistant/ Eye Care Coordinator. The Clinical Departmental Assistant/ Eye Care Coordinator’s primary responsibilities are to ensure the Clinical and Community Programs have the administrative support to fulfil their objectives and that the Eye Care Services maximize revenue. Provide administrative support for the Chief Clinical and Community Service Officer (CCCSO) and clinical and community department heads including scheduling meetings, composing letters and other projects as required. Coordinate department manager meetings and information flow including taking minutes at department head meetings as well as all other appropriate meetings as needed. Advertise for vacant clinical and community service positions and coordinate recruitment efforts. Eye Care Coordinator responsibilities include manage optical shop including working with labs to develop pricing and marketing strategies, improve communication, and ensuring scheduling efficiencies. Must have administrative experience working in a multi-disciplinary setting and use of electronic medical records with a well-organized, analytical and problem-solving skills. Excellent written and verbal communication skills as well as the ability to work as a productive member of a team is required.


Behavioral Health Coordinator

Hilltown Community Health Centers, Inc. (HCHC) seeks a Behavioral Health Care Coordinator for a mission-driven non-profit devoted to providing medical, dental, behavioral health, eye care services, and community programs to everyone, regardless of ability to pay. The candidate will effectively coordinate all Behavioral Health Services for all HCHC sites. The candidate will serve as a hub and point of contact for all patient referrals, and provide case management support as needed in referring to outside agencies as well as inter-agency resources. This position reports to the directly to the BH director. Must be well organized and experienced in providing administrative support in a Behavioral Health setting, and work well in a team. Preference given to candidates with Bachelor’s degree in social work, counseling, human services field, or relevant experience. Most importantly, must also have a strong commitment to the mission of the organization.
