Primary Care Physician

Hilltown Community Health Centers, Inc. (HCHC) is seeking a Family Practice Physician to join our growing multi-site primary care practice. HCHC is a federally qualified community health center located in Western Massachusetts. For over 65 years, our mission has been to provide accessible, affordable services to ensure the health of all Hilltown residents, regardless of their ability to pay. Established in 1950, HCHC provides primary medical care, general dentistry, mental health, optometry, and community outreach in four different sites. HCHC currently employs three talented physicians and six experienced nurse practitioners, and operates two rural primary care clinics and one urban primary care clinic, along with a school-based health center, in Western Massachusetts. Behavioral health and full dental services are offered at all sites; optometry care is also available in one location. A strong community services program provides a range of supportive services, including community outreach workers. The position involves care of adults and children, in an office-based environment, without OB or hospital responsibilities. On-call schedule shared among 10 providers. Ability to form collaborative relationships with nurse practitioners on staff a must. Occasional Saturday and holiday hours required. Working knowledge of rural services and needs a plus. EHR experience preferred. Spanish or Russian language skills are highly valued. Salary commensurate with experience with possible eligibility for state and federal educational loan repayment programs. Excellent benefits include 4 weeks’ vacation, paid sick leave, personal and holiday time, 2 weeks’ continuing education time with $2,000 for CE compensation costs, malpractice coverage, health and dental insurance, as well as payment of licensing renewal fees. Child Witness Advocate

Seeking part time Child Witness Advocate to work as part of a Community Health Team. Provide support and advocacy services to children who witness domestic violence with emphasis on both supporting children and supporting the bond between the child and the non-offending parent. The advocate will work with children, with non-offending parents, and with Domestic Violence Advocates. The Child Witness Advocate will also work in collaboration with HCHC Community Health Workers and Community Health Team in order to address the full spectrum of children’s needs, and will work closely with other community service providers and the Southern Hilltown Domestic Violence Task Force. The ideal candidate will be a team player and will be skilled at working with children as well as developing relationships and collaborating with a broad range of people in order to coordinate services and meet the needs of children. Experience working within the field of domestic violence and working with children required. The Child Witness Advocate will also assist with outreach and education efforts in collaboration with the Southern Hilltown Domestic Violence Task Force and Safe Passage. Qualifications include BA degree or equivalent. Car required and must be able to provide transportation for clients provided it is deemed safe through a safety protocol. Knowledge of community and rural services preferred. The ability to collaborate and work effectively within a multidisciplinary team is a must. Domestic Violence Program Supervisor

Seeking candidate to fill this P/T position as the coordinator of the Hilltown Safety at Home program and supervisor of program staff. Coordinate the integration of the Domestic Violence Advocates and other staff into other HCHC programs through outreach, training and collaboration. Responsible for maintaining relationships with community partners. Work in collaboration with State and Regional DV agencies, State and local police, court personnel, and other area agencies. Minimum of 5 years working in the field preferred. LICSW a plus. Able to conduct one’s self within established professional and ethical boundaries. Working knowledge of rural services and needs an advantage. Able to establish effective, positive working relationships, internal and external. Competitive salary, generous benefits.

Health Access Outreach Worker

P/T position to provide health access, information/referral services and outreach to area residents. Responsibilities include demonstrating expertise in: eligibility and enrollment rules and procedures; the range of qualified health plan options and insurance affordability programs; the needs of underserved and vulnerable populations; privacy & security standards. High school diploma or GED required. Min 1 year relevant exp. Working auto and MA driver’s license required. Must be able to maintain strict confidentiality. Working knowledge of Hilltown community and available services required. Experience providing rural outreach preferred. Community resident preferred.
