Hilltown Community Health Center uses a patient-centered model of care to provide primary medical care to our patients at three locations.
At our locations in Amherst, Huntington, and Worthington, your care team—including your provider, nurse, medical assistant, and receptionist—works together to connect you with the services you need, support your health goals, and encourage you to stay engaged in your care.
We also operate a School-Based Health Center at Gateway Regional Middle and High School, ensuring students have convenient access to high-quality primary care services.
At all of our locations you can find primary preventative and acute care for people of all ages, routine physical exams and DOT physicals, immunizations, pre-natal care, behavioral health and nutritional counseling, gynecological exams, laboratory, podiatry services and more. Our providers have specialty focuses that include HIV, diabetes, and other chronic disease management, women’s health, pediatrics and public health. Interpretation services are available in any language at your request. Our facility is welcoming and inclusive. We will see anyone regardless of their ability to pay for services and have trained staff that can help you get affordable and comprehensive health insurance coverage.
