For almost 70 years, HCHC’s mission has been to provide accessible, affordable services to ensure the health of all Western Massachusetts.
As a federally qualified community health center (FQHC) that receives some tax-payer support (approximately 20% of our total budget), HCHC must serve all patients, regardless of his/her ability to pay. HCHC is dedicated to the health and wellness of our Hilltown communities and provides a wide range of services at our five sites.
In addition to offering medical and dental services, we provide behavioral/mental health services, substance abuse treatment, eye care, physical therapy, and laboratory services. We also provide numerous community services and supports. Federal funding supports some of this work, as do insurance payments, but these are not sufficient to cover all the services we provide. We rely on grants from foundations and other sources, as well as on individual donations, to support new initiatives and all of our community programs.
Wherever you are, your team works together with your health professionals within and outside the Health Center to assure an integrated and coordinated whole-person approach. As a patient-centered medical home (PCMH) facility, we provide the most up-to-date care and regularly assess our services to improve the quality of care and service delivery.
