Sliding Fee Scale Discount Program

For almost 70 years, HCHC’s mission has been to provide accessible, affordable services to ensure the health of all Hilltown residents and beyond.

As a federally qualified community health center (FQHC) that receives some tax-payer support, HCHC must serve all patients, regardless of their ability to pay. No one will be denied access to services due to inability to pay; and there is a discounted/sliding fee schedule available based on family size and income. If you’re in need of insurance, our extensively trained Navigator staff can work with you at no charge to confidentially determine the best health plan for which you are qualified. If you do not meet eligibility guidelines for any reason, a sliding fee applicationschedule and policy is available.

We're Moving

Our Community Programs will be relocating from 9 Russell Road to our health center locations. Starting August 1, our Community Health Workers and Insurance Navigators will serve all patients and clients in the Huntington, Worthington, and Amherst health centers. This move will help better serve you and your family.
